Your Integrative Health

Privacy notice

This notice explains what the peptide pathway assessment collects, why, and how it is protected.

What we collect

Your name, email address, phone number, city and state, along with the answers you provide about your health goals, symptoms, laboratory values, medications and medical history, and whether you asked for a clinician review.

Why we collect it

To generate your educational results summary, to send you a copy of that summary by email, and — if you requested a review — so Megan Hormazdi, APRN, FNP-BC can look at your responses before a conversation.

How it is stored and who can see it

Your responses are stored in an encrypted database. Access is restricted to authenticated practice staff only; no other visitor can read any submission, including their own, once it is saved. We do not sell, rent or share your information with advertisers or data brokers.

Email and security

Practice notification emails intentionally contain no health information, laboratory values or contact details — only a first name and a link to the secure dashboard. Your own results summary is emailed to the address you provide, with your consent. Standard email is not an encrypted channel; if you prefer not to receive health information by email, do not check the consent box and ask us to remove your record.

How long we keep it

Records are retained only as long as needed for the purposes above and for any applicable professional recordkeeping requirements, then deleted.

Your choices

You may ask us at any time to see, correct or delete the information you submitted. Contact the practice at yourintegrativehealth.com.

Important limits

This assessment is for educational purposes and does not diagnose disease, prescribe medication, or establish a clinician-patient relationship. Completing it does not make you a patient of the practice. If you become a patient, the practice's separate Notice of Privacy Practices governs your protected health information.
